JOB SUMMARY

The Health Information Manager (HIM) or Medical Records Director is responsible for the daily operation and management of the Health Information Department in keeping with Center policies and procedures, professional standards, and state and federal laws and regulations for long-term care centers. The HIM establishes and follows policies to confirm that records are accurately documented, readily accessible, and systematically organized.

ESSENTIAL FUNCTIONS

Confirms medical record information is protected and kept confidential.
Protects medical record information from loss, defacing, or destruction per record retention requirements.
Verifies that health record information is filed accurately and appropriately to resident's charts, overflow records, and discharge records.
Verifies proper management and transfer of health record information as requested.
Identifies incomplete documentation (audits) and reports to individuals responsible for completion.
Implements systems developed for advising disciplines of documentation to be completed or corrected.
Mails documents to Center physicians as required for appropriate signatures and timely notification. Manages timeframes for returned signed documentation.
Monitors physician visitation based on required visitation timeframes. Interacts with physician office staff and/or physician to verify visitation occurs appropriately.
Maintains bound Master Admission & Discharge Register for admissions and discharges to and from the Center.
Verifies information on master resident face sheet is completed.
Verifies Medicare residents have timely certifications/re-certifications signed by the attending physician.
Assists with quality assessment and assurance studies.
Verifies daily census with Center Business Office.
Maintains computer database for admissions, physician orders, care plans, and MDS information.
Maintains complete MDS transmittal process that includes auditing for completeness, timeliness, and appropriate transmitting timeframes.
Observes infection control procedures and safe working procedures.
Follows Center policies and procedures.
Consistently works cooperatively with staff.
Completes special audits as requested
KNOWLEDGE, SKILLS, AND ABILITIES
Ability to read and write in English, with knowledge of arithmetic and typing speed of 60 wpm.
Knowledge of computer operation, including Microsoft Word, Excel, and data inputting.
Ability to use good judgment, make decisions, and occasionally direct the work of others.
EDUCATION, LICENSURE, AND EXPERIENCE
High school graduate or equivalent.
RHIT or RHIA preferred
Experience with Point, Click, Care software preferred but not required

One year experience as a medical records practitioner in a long-term care center preferred.
